Self-help document preparation. Coverage Rights is a self-help tool that assists you in preparing your own health insurance appeal. We are not a law firm, we do not provide legal advice, and using this site does not create an attorney–client relationship. We are not a healthcare provider and do not provide medical advice; using this site does not create a provider–patient relationship.

No outcome guarantees. Appeal outcomes depend on your facts, your plan, and your reviewers. Statistics shown on this site describe past aggregate outcomes and are not a prediction or promise about your case.

Your responsibility. You are responsible for reviewing any document you generate here before sending it, for meeting your own deadlines, and for the accuracy of information you provide.

Data accuracy. We verify insurer statistics against public sources before publishing and label unverified figures. Sources can contain errors; see our editorial standards and corrections policy, or email data@coverage-rights.com.

Payments. Paid products are one-time purchases processed by Stripe. If a generated appeal package fails to deliver, contact support@coverage-rights.com for help or a refund review.

Public datasets and CC BY 4.0. Compiled datasets offered for download on the Data & Methodology pageand marked with the Creative Commons Attribution 4.0 license may be copied, analyzed, and redistributed under that license. Attribution to Coverage Rights is required; cite “Coverage Rights, coverage-rights.com.” A link is appreciated as the practical form of attribution, not an additional license condition. The license does not cover third-party source material beyond the rights its original publisher grants.

Other content and acceptable use.The dataset license does not license the site’s editorial text, interface, software, paid appeal outputs, logo, or other brand assets. Do not submit false information, access another person’s account or private workspace, bypass security or rate limits, collect non-public user content, interfere with the service, or misrepresent Coverage Rights as the source of altered material. Automated access to public pages must respect published robots rules and reasonable technical limits.
